Steps to Apply
To begin the registration process, you must complete online credit application and submit documents to ohc-cdl@ccc.edu
Click “Apply Now” and complete the Admissions Application.
Registrants must be 21 years of age or older at the time of registration and submit a valid driver’s license.
Registration Requirements
- Obtain a court-purposed motor vehicle report from the Secretary of State for $20 – Must not be older than 30 days from the start of class (5/12/25)

Obtain a Department of Transportation (DOT) card which requires a special physical. The Concentra located at 900 E. 103rd offers our potential students this physical for $87

Obtain a Drug Screening. The Concentra offers our potential students this screening for $57 – Must not be older than 30 days from the start of class (5/11/26)

Must complete all Admissions steps prior to registration.

Must have your form of payment established prior to registration.
Please be aware that all documents are subject to prior approval before class registration. This program is based on a first come first serve basis.
